ONEcare is an integrated model of care deployed by a network of primary care, hospital, social service, and behavioral health providers that coordinates early disease identification, communication, priority access to care, and trust between the patient and their care team. By minimizing confusion and opening lines of communication, patients feel empowered to seek and continue their mental health and substance use treatment. ONEcare Networks are member-governed and partner with payers, managed care organizations, hospitals, and other Accountable Care Organizations in pay-for-performance programs which incentivize improved patient health and mental health outcomes. In 2019, Blaze Advisors organized the NC ONEcare Clinically Integrated Network (CIN) in concert with a consortium of Care Management Agencies and Advanced Medical Homes who leverage the ONEcare Care Optimization System which includes the following modules:

• Clinical Intelligence Analytics: The data aggregation, normalization, analysis, and reporting engine.  Includes AI and Machine Learning for risk stratification.
• Care Management System:  The case planning, documentation, task management, and alerting system for case/care managers.  
• Care Coordination Platform:  Tracks e-referrals and communications between the care team.  Includes screening, care alerts, and ability to transfer clinical information.
• ONEtouch Patient Engagement Mobile App:  Downloadable app that allows patient to virtually communicate with their care team/manager, schedule appointments, and access non-ED crisis services.  Includes automated wellness checks and health literacy content.

The ONEcare Care Optimization System meets all the functional requirements of Tailored Care Management and Blaze Advisors is working with CMA superusers to customize the CMA-specific forms, alerts, and reports to NC DHHS specifications.
